Service Region: Anna serves clients in Rochester, New York region including Monroe, Ontario and Wayne Counties. A nominal travel charge may be necessary depending on your location.

Pre-massage questionnaire: On your first visit, I will ask you to complete a brief Personal Data Form (page 1 and 2 are available below) and a Symptomatic Areas Form (the form for your gender is available for download below).
